PSU Equipment Loan Form

I redesigned an outdated Equipment Loan paper form from Portland State University to be an accessible, fillable PDF form. As the form included a legal agreement that did not meet accessibility standards in its original form, I consulted with an attorney to rewrite the language in a way that satisfies the legal requirements while still being accessible. My new version of the form is much more intuitive, and meets PDF Accessibility Standard for WCAG 2.0.
